Edge Plus:
- Provides full dental coverage:
- Two plans available
- Choose any dentist
- Employee groups, totally voluntary enrollment
- Minimum enrollment of 3 full time employees
- No employer contribution required
- Major restorative procedures such as bridges, crowns, dentures
- Orthodontia benefits
- Can combine Edge Plan A with Basic Plus in same enrollment
- Plan B available without the Orthodontia benefits
Basic Plus:
- Offers very low rates:
- Choose any dentist
- Employee group, totally voluntary enrollment
- Minimum enrollment of 3 full time employees
- No waiting periods
- Coverage of preventive/restorative procedures; extractions, oral
surgery, and root canals
- Can combine with Edge Plus Plan A in the same enrollment
Unique Edge:
- Extremely competitive rates with employer's choice between two
"true group" plans with different levels of benefits:
- Choose any dentist
- Minimum enrollment of 2 full time employees
- Both plans include all dental procedures and orthodontia
- Participation requirement varies from 75% to 100%, based on total
eligible employees
- Employer contribution requirement varies from 50% to 75%,
depending upon group size.
- $1,500 maximum annual benefit per family member
Association:
- Provides group insurance rates for accredited associations
- Choose any dentist
- 50 or more members must enroll
- Totally voluntary enrollment
- Two plans available; members have a choice of benefits and rates
- Payment may be by monthly membership deduction or individual
monthly bank draft
